Scar revisions range from the simple excision and reclosure of an unsightly
scar, to the complicated redirecting and release of scar tension and
tissue. In addition, techniques such as dermabrasion, laser resurfacing,
or punch grafting may be used to optimize the final result. Dr. Alsarraf
cautions that not all scars can be removed in their entirety, but has
himself published a teaching article on one advanced technique for scar
revision, entitled "Geometric Broken Line Closure," in a special
issue of the Facial Plastic Surgery Clinics of North America dedicated
to scar revision advances. This article, along with Dr. Alsarraf's other
publications, is available for your review at The Newbury Center.
Most scar revisions take less than 1 hour and can be performed in either
the office or an out-patient hospital setting, depending on patient preference.
Rhinoplasty is the art of altering the form and function
of the nose, and remains one of the most difficult plastic surgery operations
performed today. Examples of poorly done nose jobs abound in the public
eye and media. For this reason, Dr. Alsarraf completed his American Academy
of Facial Plastic Surgery (AAFPRS) training with Calvin M. Johnson, Jr,
the author of the landmark text "Open Structure Rhinoplasty" and
one of the fathers of modern rhinoplasty.
Dr. Alsarraf's goal is to create a natural appearing result that maintains
the patient's nasal function. To achieve this end, the underlying nasal
structure is reshaped to provide a more attractive and pleasing profile
that does not appear operated upon or unnatural. Dr. Alsarraf has himself
published several articles and chapters on specific rhinoplasty techniques,
including in depth discussions of "The Post-Traumatic Rhinoplasty" and "The
Saddle Nose Deformity" for special issues of the Facial Plastic
Surgery Clinics of North America. These articles, along with Dr. Alsarraf's
other publications, are available for your review at The Newbury Center.
The rhinoplasty procedure takes approximately 1-2 hours and is performed
in the out-patient hospital setting. No nasal packing is placed, and
most patients experience minimal pain or bruising.
Obagi Nu Derm
Because even the best surgical procedures cannot correct all of the
signs of aging that time and the environment have caused on the texture,
coloration, and quality of the facial skin, we recommend the use of a
Retin-A based skin therapy regimen for many of our patients. In contrast
to more invasive procedures such as laser resurfacing or chemical peels,
a maintenance program such as Obagi is a safe and reliable method for
diminishing fine lines and wrinkles, smoothing the skin surface, and
eliminating uneven or blotchy discoloration. Dr. Alsarraf also performs
skin resurfacing procedures, but often recommends that his patients first
start on a Retin-A based system for several weeks prior to undergoing
such treatment.

Hocus Pocus
"In addition to the Obagi Nu Derm skin therapy regimen, which works
wonders at erasing the signs of sun damage and aging from the skin of

Dr. Ramsey Alsarraf specializes in cosmetic surgery of the face and neck.
Many of the procedures that he performs are designed to rejuvenate the
aging face, such as the facelift, browlift, or blepharoplasty. In addition,
he performs a wide range of other cosmetic facial plastic surgery procedures
and has a specific interest in rhinoplasty and nasal reconstruction. Dr.
Alsarraf has himself published many technical articles and book chapters
on procedures such as the deep plane facelift, rhinoplasty, malar or cheek
augmentation, and scar revision. The Newbury Center also offers a full

Although The Newbury Center specializes in cosmetic
facial plastic surgery procedures such as the facelift, browlift, blepharoplasty,
and rhinoplasty, Dr. Alsarraf also performs a wide range of general facial
plastic surgery procedures as well. These range from focused techniques
such as ear reconstruction or pinning (Otoplasty), to the removal and
closure of facial skin cancer defects. In addition, straightforward procedures
that can be performed include mole removal, earlobe reduction, dermabrasion,
septoplasty, and minor cosmetic surgery revisions.

There are several methods used for augmenting or increasing the fullness
and projection of the lips. These range from the simple injection of
collagen in the office setting, to the insertion of a permanent Gortex
implant. For patients who desire something in between, Dr. Alsarraf recommends
the use of Alloderm, an acellular human dermis graft, which lasts longer
than collagen but avoids many of the potential complications of a permanent
synthetic graft.
Lip augmentation takes 1/2 to one hour and is performed in either the
office or the out-patient hospital setting, depending on the procedure
that is chosen.
The facelift is designed to rejuvenation the sagging shapes of the aging
face while maintaining a natural, unoperated upon appearance. Dr. Alsarraf
specializes in the deep plane facelift, a surgical technique that creates
a longer lasting and more natural result. Other, older forms of the facelift
can create a pulled or wind blown appearance or produce results that
fade quickly over time. Dr. Alsarraf has published a detailed account
of the deep plane facelift entitled "The Facelift: Technical Considerations," in
a special Rhytidectomy issue of Facial Plastic Surgery. This article
and Dr. Alsarraf's textbook The Aging Face: A Systematic Approach, which
also focuses on the deep plane facelift technique, are both available
for your review at The Newbury Center.
The facelift procedure takes approximately 3-4 hours and is performed in a
hospital setting. Many patients elect to have a browlift or blepharoplasty
at the time of their facelift, in order to maintain a balanced facial rejuvenation.

After graduating from Harvard University and the Yale University School

surgery. Dr. Alsarraf, a member of the AAFPRS, is board certified by
the American Board of Medical Specialities (ABMS) and the ABO, with subspecialty
training in cosmetic facial plastic surgery. He is a nationally respected
author, editor, and lecturer, and has published over 30 articles and

chapters, in addition to co-authoring the recently published full length

Collagen may be injected into the lips, deep creases or folds of the
face, to create a fuller or more youthful appearance. Dr. Alsarraf prefers
the use of human acellular collagen (Dermalogen or Cymetra) rather than
the older, bovine collagen, because no skin testing is required as it
is not associated with allergic reactions. Collagen lasts less than six
months in most patients, but can be repeated indefinitely. Other options
for lip or facial augmentation include the use of Alloderm or a Gortex
implant, if a more permanent result is desired.
Collagen injections take only a few minutes to perform in the office
setting. Topical anesthesia with a local anesthetic block is often employed
for patient comfort.
The use of synthetic implants is a safe and effective means of augmenting
various regions of the face and achieving a more youthful or attractive
appearance. Dr. Alsarraf prefers the use of Silastic and Gortex implants
for such augmentation because they have proven to provide a natural appearing
result and are well-tolerated in most patients. A chin implant may help
balance facial symmetry at the time of the rhinoplasty procedure, while
a cheek (or malar) implant may increase the projection of the cheekbones
and rejuvenate the midface region. Dr. Alsarraf has himself published
a technical review of one method of cheek augmentation, entitled "Malar
Augmentation with Self-Drilling Single Screw Fixation," in the Archives

Placement of a chin or cheek implant takes about an hour and is generally
performed in an out-patient hospital setting.
The browlift attempts to restore the youthful position of the eyebrow,
as well as to eliminate the deep wrinkles of the forehead, the area above
the nose, and between the eyes. Although the endoscopic browlift is appropriate
in some patients, many patients do best with the open browlift, which
has stood the test of time. In any case, the incisions for the browlift
are generally well hidden behind the hairline in most patients.
The browlift procedure takes approximately 1 hour and is performed in an out-patient
hospital setting. It is often performed in conjunction with the facelift,
so that a balanced rejuvenation of the overall facial aesthetic is produced.
Botox has been used for several years to decrease the fine lines and
wrinkles of the forehead and between and around the eyes by freezing
the small facial muscles that create them. It is a safe and effective
method of treating these wrinkles, however, the effects are temporary,
lasting less than 6 months in most patients. The browlift is the definitive
treatment for the aging lines and wrinkles of the brow and forehead.
Dr. Alsarraf has been featured on several local and national television
features, discussing the use of Botox for facial rejuvenation, including
the CBS News Magazine Program "48 Hours."

The blepharoplasty rejuvenates the aging appearance of the upper and
lower eyelids. Dr. Alsarraf performs the transconjunctival or incisionless
lower lid blepharoplasty in most patients because it limits the visibility
of skin incisions and is an effective method of addressing the bulging
fat of the lower lids. The upper lid blepharoplasty is often performed
in conjunction with the browlift to correct the aging eyebrow as well
as the redundant tissue of the lid itself.
The blepharoplasty procedure takes 1/2 to 2 hours and is performed
in an out-patient hospital setting. Like the browlift, the blepharoplasty
may be performed at the time of the facelift in order to achieve a uniform
and balanced facial rejuvenation.
